Having a giveaway like this is a cool idea, and a savvy move to increase your subscriber count. I was a little wary of supplying personal data to you up front, though. I can see how you’ll need a name (well, maybe) and shipping info from the winner, but is it necessary to collect this info from everyone who wants a shot at the book?
Don’t get me wrong – I love your blog…but what if my info somehow gets into the hands of fundy Christians and they mail me tons of tracts and send prayer warriors to my doorstep?

I highly doubt someone will hack into the form and get your address, but I suppose anything is possible these days!
